The due date for completion and return of this application is Monday, February 23rd by 5pm CST. This packet contains an introduction letter with instructions on how to complete the enclosed application, including the statistics specific to your library that you will need. It also includes a pre-addressed, stamped envelope for return of this application to SWAN. We will not be opening the applications but sending them directly on for processing. However, we need to tally the receipt of these applications to ensure 100% compliance.
